Banquet Form Required From ALL

Hello Everyone,

Our 2018 South Lyon Robotics Awards Banquet is now being held on Wednesday, May 30, 2018 from 6:00 pm - 9:30 pm instead of its original date of Thursday May 31, 2018.

For those of you (Students, Parents, & Mentors) planning on attending, I have enclosed the "2018 South Lyon Robotics Awards Banquet Attendance Form (20180506).pdf" for your use.

Please fill in the form and mail it back to me NO later than Monday, May 14, 2018. In addition, please include with the returned form a check (made payable to "South Lyon Robotics Boosters") to cover the cost of the banquet for any adult participants that are NOT registered as a MENTOR on the team. The 2nd page of the enclosed form contains an example of how The Martin Family would fill out the form (includes 2 REGISTERED STUDENT MEMBERS, 1 REGISTERED ADULT MENTOR, 1 child under the age of 15 years, & 1 adult parent).

If your completed attendance form requires no monetary payment, PLEASE MAIL IT BACK TO ME ANYWAY. I need the total count of people attending the banquet for catering preparations.
